Rhinoplasty is surgery that changes the shape of the nose, and is one of the most frequently performed surgical procedures in plastic surgery.
Rhinoplasty can reduce or increase the size of the nose, change the shape of the tip or the back, narrow the nostrils or change the angle between the nose and upper lip.
Also it corrects congenital problems, trauma and some respiratory problems.

Ideal candidates for rhinoplasty are those looking for improvement, and not absolute perfection, in their appearance. If you have a good general state of health, are psychologically stable, and realistic in your expectations, you will be a good candidate. Rhinoplasty will improve your appearance and self-confidence, but you will not look like someone else and people will not treat you differently. Before deciding whether to perform rhinoplasty, think about what you want to achieve and discuss it with your plastic surgeon.
Age should also be taken into account. Most plastic surgeons prefer not to perform a rhinoplasty until you have not gone through the “growth spurt” that is, until 14 or 15 years for girls. It is also important to consider the emotional and social adjustment of adolescents, and be sure that they, and not their parents, are the ones who want to undergo rhinoplasty.
Rhinoplasty is always done in an operating room, within a clinic or hospital. Normally, the patient stays at the clinic the night after surgery. Rhinoplasty can be performed under local anaesthesia with sedation, or what is more common, with general anaesthesia, depending on the complexity of the surgery and the preferences of your surgeon.
If done with local anaesthesia and sedation, the patient feels relaxed and the nose is insensitive to pain; with general anaesthesia the patient is asleep during the operation.
A rhinoplasty usually lasts between 1 and 2 hours, although complex cases may take longer. During surgery, the skin of the nose is separated from its support, composed of bone and cartilage, which is sculpted to the desired shape. Finally the skin is redistributed.
Many plastic surgeons perform this procedure from within the nose, making small incisions inside the nostrils; others prefer to do an open rhinoplasty, this is especially true for more complex cases, in which a small incision is made in the columella.
When surgery is completed, a plaster cast or other material, which will help maintain the new shape of the nose, is placed. Nose plugs are also placed in both nostrils to prevent bleeding and to stabilize the nasal septum.
After surgery, especially during the first 24 hours, you may feel swelling in the face, have a nose and headache, which can be subsided with medication. You must remain lying with the head elevated during the first day (except for going to the bathroom).
You will notice that the swelling and bruising around the eyes increase to a maximum at 2 or 3 days. A cold compresses decreases swelling and will make you feel better.
Most bruising and swelling will disappear in about 2 weeks (minimum swelling, almost perceptible only by your surgeon may last a few months).
It is normal to see slight nosebleeds for the first few days. You should not blow your nose hard for a week or so, until the tissues heal. If you have nose plugs, they will be removed within 1 or 2 days, after which you will feel much better.
After a week, or at most two, the cast and sutures will be removed.
Most patients who undergo rhinoplasty are well in 2 days, being able to return to work or school in a week or so. However, they should not perform some activities for a few weeks. Although your plastic surgeon will provide specific instructions, some recommendations are to avoid strenuous activities (jogging, swimming, doing push-ups, …) for 2 to 3 weeks, to avoid blows to the nose and sunbathing for 8 weeks, to wash your face and apply makeup carefully.
You may wear lenses as soon as you want, but if you wear glasses you should take certain precautions.
Your plastic surgeon will see you during the months following surgery to see the evolution. If you have any trouble between visits, or any questions about what should or should not do, do not hesitate to call.
When rhinoplasty is performed by a qualified plastic surgeon complications are infrequent and minor. However, there are always chances of complications such as infection, nosebleed, or reactions to anaesthesia. Following the instructions of your plastic surgeon carefully, both before and after surgery, can minimize risks.
Sometimes after surgery small red spots, which are small broken vessels may appear on the skin; generally they are very small, but they are permanent. There will be no problems with scars because the incisions are inside of the nose; even in open rhinoplasties, the scar it is imperceptible. Approximately in 1 out of every 10 cases is it necessary to perform a surgical revision to correct minor defects. Such cases are unpredictable and happen even to patients of plastic surgeons that have a lot of experience.
During the first few days after surgery, when your face is stillswollen and bruised, it’s easy to forget that your appearance will improve and many patients may feel somewhat depressed; it is a normal feeling. Day by day the appearance improves and the patient begins to feel happier. After 1 or 2 weeks, you will not appear to have undergone surgery. The improvement is progressive and gradual, although minimal swelling can be maintained for a few months. The final result is obtained in about 1 year.
You cannot pass judgment on the result of a rhinoplasty until several months have elapsed, even a year. This is the time needed to stabilize tissues and scars. The intention of rhinoplasty is that the “new” nose does not attract attention, and is fully integrated into the overall context of the face by producing a harmonious and pleasant whole, i.e. it should not be detected that has been tampered with.
Occasionally, either because of severe initial anatomical issues or because of disorders in tissue healing or for postoperative trauma, the results may not be perfect (asymmetry, irregularities in the nasal contour, etc.) In that case, surgical retouching will be done after six months.
